Adult Family Care Receives Prestigious Accreditation

Adult Family Care (AFC), a program of Somerville-Cambridge Elder Services, recently received the highest-level accreditation from the National Committee for Quality Assurance (NCQA). AFC earned a three-year Accreditation of Case Management for LTSS (Long Term Supports and Services).  

NCQA evaluates organizations that plan and manage personalized care and services for people experiencing difficulty completing self-care tasks as a result of aging, chronic illness or disability. Its accreditation standards are developed with input from researchers in the field, the LTSS Advisory Committee and standing committees, both purchasers and operators of LTSS programs, state and federal regulators and other experts. 

AFC goes through NCQA’s rigorous accreditation process every three years. This ensures the program meets its goals and provides excellent care — NCQA only awards the status of Accredited–3 years to organizations that demonstrate strong performance of the functions outlined in the standards for Accreditation of Case Management for LTSS. 

“Being accredited by NCQA proves to our participants and partners that AFC is committed to upholding high standards,” said Program Director Nina Cohen. “Our work strives to put participants and caregivers, and their needs, at the forefront.” 

AFC serves eligible adults who cannot live alone due to a medical diagnosis. The program’s person-centered care model helps participants and caregivers create holistic, individualized plans with the goal of achieving high quality care at home. Caregivers benefit from tax-free stipends, paid time off and ongoing support, while participants are cared for by a trusted person. 

AFC’s dedication to its participants is reflected in its survey data. In the latest participant survey, 99% of respondents said their caregiver has the skills and knowledge to provide the care they need. More than 95% said they would recommend AFC to a friend.  

“AFC is designed to help people receive the best care at home. Our program is uniquely suited to helping friends and families access the tools they need to care for a loved one and feel supported while doing so,” said Cohen.
