What do we do?
We provide specialized home care services to individuals that cannot live on their own due to chronic illnesses and disabilities.
AFC gives families the tools they need to help their loved one live at home and age in place. These tools include monthly home visits from a nurse and social worker, a tax-free caregiver stipend, ongoing training and teaching on the individual’s disease process, caregiver stress, and coordination of community resources. Our AFC caregivers receive support from our AFC team to provide the best care at home for their loved one.

We have access to many resources such as Fall Prevention, Caregiver Support groups, Music & MemorySM. We value collaboration with our AFC participant’s physicians and providers. We have masters level social workers with a growing team of licensed independent social workers. We have 9 registered nurses on staff.
