Adult Family Care serves eligible adults who cannot live alone due to a medical diagnosis. The clients we serve have a wide range of diagnoses that include neurodevelopmental disorders, dementia, and more.
Adult Family Care is a MassHealth-funded program. To qualify, the person receiving care must:
- Be 16 or older
- Have MassHealth Standard, CommonHealth, Senior Care Options (SCO), OneCare, or PACE
- Be unable to live alone due to a medical diagnosis
- Require assistance with at least one of the following: bathing, dressing, toileting, transferring between positions, ambulating, or eating
The Caregiver must live with the participant and cannot be their spouse or legal guardian.
