To make a referral, you will need the following information.
Person needing care (client, consumer, or participant)
Name, address, phone number, MassHealth ID or Social Security number, their medical diagnosis, activities of daily living they need help with and an emergency contact, who lives in the home.
Caregiver (if there’s already one)
Name, address, phone number and whether they are the legal guardian.
